You may have noticed that a lot of the things your podiatrist is offering you for your bunions are helping to relieve the pain, but not helping to treat the issue. Bunion surgery with the NHS requires a lot of downtime and usually is not an option for many who lead busy lives.

Mr Andrea Bianchi is our Orthopaedic Consultant Surgeon at The Private Clinic. Andrea Bianchi is the founder of PBS, the team who developed the minimally invasive bunion surgery in 1996 and has supervised over 50,000 surgeries since.

Minimally invasive keyhole bunion removal surgery is able to remove bunions from the foot with a quick mini-invasive surgical procedure performed under local anaesthetic that doesn’t involve any fixative devices such as screws, plates or wires. After the treatment patients are able to walk immediately with no crutches and get back to their everyday activities, with no downtime and a lot quicker than traditional bunion removal surgery. The scaring post-procedure is also minimal as no z shaped cut is used.

Mr Bianchi uses small surgical dental burrs which are inserted through a very small 2-3 millimetre incision. These burrs are then guided using a special radiological device which allows him to see the bones and surgical burrs without the need for larger incisions. Dr Bianchi will then create a series of small and precise fractures to correct the bunion deformity and modify the bones of the foot which is known as a geometric fracture. These geometric fractures do not require fixative devices such as screws or pins which force the bone into place. The geometric fractures created are left to heal naturally without the strain of being forced into a ‘pre-set standard which may not be natural for the patient. After the procedure, the patient is encouraged to walk on the foot which means the foot can heal according to the patient’s load-bearing needs and the correct physiological readjustments.

Our minimally invasive bunion procedure is increasingly popular with patients with demanding jobs, busy family lives and generally anyone who does not want to commit to a 6 month period of downtime. Patients will have their treated foot taped using a personalised bandaging technique allowing for the physiological healing process to restructure the foot without the need for the patient to keep it rested at all times. Patients will be required to wear a specialised orthopaedic shoe for up to 20 days before then returning to the clinic to have the bandaging replaced and reduced and many patients are then able to wear wide-fitting trainers to return home.

How is minimally invasive bunion surgery different to other bunion correction techniques?

The extensive downtime required after traditional bunion surgery has meant that many people choose to not go ahead with surgery and instead rely on remedies such as bunion pads, paracetamol and purchasing wide shoes which can come at quite a cost.

Here at The Private Clinic of Harley Street London we are excited to be able to offer a minimally invasive bunion removal surgical procedure that has been inspired by keyhole surgery techniques.

Developed by Orthopaedic Consultant Surgeon, Dr Bianchi who will be performing the procedure at our London bunion clinic. This is bunion surgery with a difference as patients are able to walk straight after surgery, have minimal downtime and more importantly no fixative devices such as screws or wires which are usually what causes patients discomfort post-surgery.

Dr Bianchi who has been 15 years’ experience and performs 1,200 minimally invasive bunion procedures performed each year explains to us how minimally invasive keyhole bunion surgery at The Private Clinic is different to other bunion surgery techniques;

Why we don’t use metal screws

Introducing an external element such as metal screws into the body will always be very invasive. For example, the metal fixative devices used in bunion surgery can create problems such as the joints in the foot not being able to find a natural posture, which can result in a huge amount of discomfort post-surgery. Because of this, in some cases a second operation may be required to remove them. In addition, the healing from this kind of surgery can take a prolonged period of time and in some instances crutches are needed before the foot is able to bear weight.

In order to address these issues with standard bunion surgery, we have developed a more natural and biological method without using fixative devices (screws, plates or wires). Through my research over 20 years, it has been proven that self-stabilizing geometric fractures can be made on short-bones (without the need for immobilisation) with long term results. This not only means that healing and recovery is much quicker and easier, but that greater joint mobility is achieved after the surgery, compared to standard techniques.

Each bunion procedure carried out at The Private Clinic is customised to the patient, and this is not a ‘one-size fits all’ approach. The bone is fractured, re-aligned and bandaged in such a way that creates the perfect result for the patient’s anatomy and better comfort and joint articulation. The absence of screws means that the joint is not fixed in one place, it can move freely.

In addition, recovery from the minimally invasive method does not require the immobilisation of the foot so the patient can go back to walking immediately post-surgery without the need for crutches. Thanks to the special bandaging technique and supportive shoe, the patient is able to walk unaided straight after the operation.

Why is the bunion surgery technique so innovative, and what does the bunion procedure involve?

The PBS technique quashes the concept that every fracture needs to be immobilised in order to heal. After being realigned, the fractures made during surgery are left free, so the foot heals according to the individual’s load-bearing needs and not according to a ‘pre-established’ standard.

Creating series of geometric fractures (a small, precise fracture used within the PBS technique) without forcing the bone into place using fixative devices, allows the foot to heal naturally. As the PBS technique does not force the bone to repair, the bones and joints heal in a natural position, which results in greater functionality and comfort in the joint.

Small, surgical, dental burs are used to correct the deformity and modify the bones of the forefoot. These are inserted through small, approximately 2-3-millimetre incisions in the skin. The surgeon guides the bur using a special radiological device to help him “see” the bones and the burs. The most important innovation of the technique is that it does not use fixative devices (screws or wires). This allows the fractures to heal according to the requirements of the individual’s foot.

After surgery, the foot is bandaged and taped using a technique specifically developed by my medical team and this must be worn for at least 20 days.

What exactly is a geometric fracture?

A geometric fracture refers to way the bones are carefully fractured with precision and then realigned into a straight position. The fractured bone is then bandaged without the need for screws to keep it in place, promoting the natural healing of the foot.

Are you able to guarantee 100% a solid joint between the bones that has only been cut with its method of firmly wrapping the foot?

No surgery is 100% guaranteed. However the PBS this method has a 98% success rate.

Many people wish for bunion treatment but are often unable to have the traditional surgery due to the long downtime that is involved. Our new minimally invasive bunion treatment available exclusively at The Private Clinic allows patients to have their bunion removed and continue with their day-to-day activities straight after surgery.

Bunions are the boney lump that is commonly found on the side of the foot or sometimes the base of the big toe and as well as being unsightly they can also make wearing particular types of shoes difficult and be painful for many.

Previously, bunion removal surgery has required patients to allow for approximately 6 weeks downtime where they would not be able to walk, work or play sport for a period of time. This impact on our everyday lives is often not even an option and many choose to reject surgery in favour of non-surgical alternatives such as bunion pads and specialist shoes to help to temporarily alleviate the pain. Although these options are great at providing some relief to patients’ lives, they do not treat the bunion or help in any way to decrease the size.

Our minimally invasive bunion removal procedure has changed the way that bunions are treated. Our treatment allows the patient to walk shortly after surgery without the need for crutches meaning patients can return to their everyday activities a lot quicker compared to the traditional surgery. Our minimally invasive procedure does not use any fixative devices, such as wires, screws and plates and has a 98% success rate achieving outstanding results with minimal scarring.

How does Minimally invasive bunion surgery work?

Minimally invasive bunion surgery at The Private Clinic uses keyhole surgery techniques and a specialist radiological device.  Small 2 to 3 mm incisions are made within the skin allowing the surgeon to insert tools to cut the bone (dental burrs) which are able to reshape the foot to correct the deformity. You will have bandages applied to your foot which will need to remain in place until you return in a few weeks’ time for your post-op check.

What is the downtime?

Post-operative pain after minimally invasive bunion surgery is very low and you will be provided with detailed aftercare instructions to help you during your period of recovery.  Shortly after your procedure once your local anaesthetic has worn off you will be encouraged to walk on your foot. Your surgeon will encourage you to do this regularly as it will help your bones to heal correctly. You will not require crutches or physiotherapy but you will be provided with a specialised orthopaedic shoe that you will be recommended to wear up until you visit again for your post-op check-up where in most cases you will be advised to start wearing comfortable shoes.
