How to carry out a mole check and protect your skin in summer

Most people with moles know they should check them regularly for signs of skin cancer, but how many of us actually know what we’re looking for?

It’s important to get to know your skin and what is normal for you, so that you notice any changes, particularly as skin cancers rarely hurt and are much more frequently seen than felt.

While not all skin cancers are deadly, melanoma, the most dangerous and third-most-common kind, is extremely deadly if not found early.

If you don’t regularly keep track of the moles on your body Dr Almaani, Consultant Dermatologist at The Private Clinic of Harley Street, has provided tips and insight on how to carry out regular mole checks and protect your skin this summer.

Here’s everything you need to know:

How to check your moles

It is important to get into the habit of self-monitoring your moles, and to always report any changes to your GP who will then refer you on to a dermatologist if the diagnosis is uncertain, or if there is a suspicion of cancer.

Keep track of your moles

It is much easier to spot a change if you are checking your moles regularly. It is important to know where on your body you have moles, and to be aware of any changes as you age. You can always ask your partner, or a friend to check the moles you cannot see clearly yourself. Photographs are useful and can help keep track of moles particularly when you have many.

It is normal to have between 10 and 40 moles by the age of 40, and if you have always had moles and one of them starts to change, that is the time to make an appointment with your GP. It is also important to monitor any new moles which may appear.

What to look for

When checking your moles, the general advice is that, if you notice any changes or unusual growths then it is best to seek medical advice early on. Do not ignore the warning signs which can manifest in the form of itching, pain, bleeding or a change in colour.

It is easier to follow the ABCDE rule by monitoring for changes in:

•        Asymmetry – when two halves of the moles do not look the same

•        Border (irregularity) – blurred or jagged edges of the mole

•        Colour – more than one shade present in the mole

•        Diameter (size)- if you notice a change in the size of the mole

•        Evolution – change over time, itching, bleeding).

Preventative measures you can take

Understand the difference between UVA and UVB

UVA forms the majority of UV light and contributes to skin ageing and the development of skin cancer as it penetrates deeper into the skin (the main UV rays emitted by sunbeds). UVB is more intense and leads to the redness and sunburn that is indicative of some degree of DNA damage which in turn can potentially lead to genetic mutations and skin cancers.

Apply sun cream

This may seem like an obvious one, but people often overestimate the protection they are getting from their sun cream, and will also often opt out of wearing it on cloudy days. This is not a good idea because the sun’s rays can penetrate cloud, and can have damaging effects all year round.

Using the published teaspoon rule, an average of 6 teaspoons should be sufficient to cover an adult body (half a teaspoon to each arm, head and neck, one teaspoon to each leg, chest and back).

To increase sun protection it is best if sun creams are applied 20 minutes before wearing clothes/going out, and use a higher SPF. Ideally another layer should be applied an hour later for maximum effect, and every few hours thereafter particularly when sweating and swimming.

Don’t forget lip protection as sun damage and cancers can develop in this area.

Is your mole harmless or life-threatening? Expert reveals 7 types of growths and what they could be trying to tell you

Some things are part of who we are, like fixed earlobes or square nails.

That’s how I’d always seen the mole on the back of my left thigh.

15 years ago it was a neat, round, slightly raised brown dot, like something you’d make with the tip of a marker pen.

But in the last couple of years it’s been changing.

The border now has jagged asymmetrical edges and it’s grown in size fourfold, its surface turned scaly.

Instead of an even brown, its pigment is now darker in parts.

I knew these weren’t good signs and that my history of sunburns as a child – I grew up in Australia – put me at increased risk, even with my olive skin.

As a fully paid-up member of the vain hypochondriacs club I went to see leading dermatologist Dr Noor Almaani, who specialises in moles, both malignant and benign.

She works at King Edward VII NHS Hospital in Windsor and The Private Clinic in Harley Street, London where I stood in her surgery in my knickers while she looked at every mole on my body with a ‘dermatoscope’.

This is a hand-held magnifier that doctors place over moles to see what’s really going on in the deeper layers of the skin.

While Dr Almaani is almost certain the mole on my leg is a harmless ‘serborrheic keratosis’, she thinks it’s best cut out and checked out.

‘The fact that it has doubled in size in the last year, is asymmetrical and that you only have one like it on your whole body is enough reason to have it removed and tested,’ she said.

Like me, almost all normal adults will have around 10-40 moles of varied shapes, colours and textures on their bodies by their 40th birthdays – very few of which turn out to be cancerous.

But how can we know that what we have really doesn’t need to be checked by a specialist for skin cancer?

Writing for Healthista, below, dermatologists describe reveal the four main types of benign lumps, bumps and growths that can develop on skin with age – and exactly what to do about them…

1. Benign moles

What are they?

Small coloured spots on the skin that can be flat or raised, smooth or rough and may have hair growing from them.

‘Benign moles are incredibly common,’ Dr Almaani said.

‘The vast majority we see are what we call “typical moles” so their borders are smooth and they’re symmetrical in shape and almost the same colour all over – this is normal’.

What can be done?

The NHS won’t remove benign moles, unless there are symptoms such as itching, pain, a recent change in colour and size or asymmetry.

A family history of skin cancer, a history of sunburn or sunbed use, inflammation or bleeding that could signal cancer are among other reasons they might be removed.

However, it’s possible for benign moles to turn cancerous later in life, Dr Almaani said.

‘You need to keep checking,’ she added.

Privately, benign moles can be removed.

‘If the mole is raised it can be surgically shaved off in a procedure done under a local anaesthetic that takes about half an hour,’ she continued.

‘This might leave a small scar a bit lighter than the skin surrounding it.’

If the mole is flat, there’s little that can be done except cutting it out, also a quick procedure under local anaesthetic.

A dermatologist cuts into the skin around the mole and removes it, then stitches the skin together again, leaving a scar that’s usually a straight line.

‘The only reason you can get a mole removed on the NHS is if your doctor suspects a malignancy and refers you for treatment, in which case it will usually be biopsied or removed and sent for testing,’ she said.

What am I looking for?

‘You’re looking for change,’ Dr Almaani said.

Check your whole body regularly and know where you have moles so you can be aware of any changes as you age.

Remember it’s normal to have between 10 and 40 moles by age 40, Dr Almaani advised.

‘If you have always had moles and one or some of them start to change, report that.

‘But likewise, if you have never had moles and develop one in your 40s, tell your doctor.’

Never ignore red flags such as itching, pain, discharge, bleeding or a change in colour, texture or size, she added.

Also, be aware of the ABCDs of skin cancer, from Cancer Research UK – having even one of the following features of a mole warrants getting it checked.

THE ABCDs are:

Asymmetry – this is when the two halves of your mole don’t look the same

Border – the edges of your mole are blurred, jagged or irregular

Colour – there’s more than one shade present in your mole

Diameter – your mole is wider than six millimetres (about the size of a pencil eraser)

Skin cancer types

Though the differences aren’t always obvious to the naked eye, a specialist can identify the type of skin cancer to determine the treatment you need.

Basal cell carcinoma (BCC)

‘By far the most common cancer in the body affecting 75 in 100 cases, this is a mild type of skin cancer and in 99.99 per cent of cases is limited to the skin’, Dr Almaani said.

‘A dermatologist can scrape, freeze or cut them out but there is usually no urgency in treating these’.

Squamous cell carcinoma (SCC)

This makes up about 20 per cent of skin cancers and while it doesn’t often spread, it still should be treated urgently, according to Cancer Research UK.

Malignant melanoma 

This is a rare type of skin cancer and in the UK, most are found and treated at an early stage.

Under guidelines from the National Institute for Clinic Excellence, if a GP suspects either squamous cell carcinoma or malignant melanoma, you should be seen by a specialist within two weeks on the NHS, Dr Almaani said.

A biopsy might be taken, or the whole thing might be surgically removed.

‘We usually prefer to take out the whole mole to assess the depth of the cancer. We might also take a biopsy of the lymph nodes to check the cancer hasn’t spread.’

For most, surgery will be enough because the cancer will not have spread.

‘If it was a deep cancer, we will usually follow the patient every two months for about three years to ensure it doesn’t come back.’

If the cancer has spread, chemotherapy and radiotherapy along with medication will be prescribed.

‘I can’t stress enough how good survival is for skin cancer so see your doctor early if any mole changes,’ Dr Almaani said.

WHAT TO DO 

Ask for a referral if you’re in any doubt.

According to NICE guidelines, if your doctor has any suspicions or isn’t sure, he or she should refer you to a specialist within two weeks for a biopsy or removal and testing.

  • When is a mole dangerous?

    We recommend checking your skin every few months for any new moles and changes in existing ones. Any new moles that appear after the age of 30 should be checked thoroughly by a professional as their risk is significantly higher. A helpful way to remember what to look for in determining a dangerous mole is to use the ABCD method:

    Depending on the size of your mole, its location and the method used to remove it, you may be left with a small scar. Your dermatologist will always work with you to ensure that any blemishes are minimal and will advise you on your risk of scarring during consultation and treatment.

  • How much does Mole Checks & Removal cost?

    The cost of an appointment with our Consultant Dermatologist is £200. A full mole check cost is £200 (included in the consultation price) and, depending on the treatment course offered by the Consultant Dermatologist, mole removal will cost approximately £600. If you require a biopsy, the following pricing guidelines apply: Diagnostic skin biopsy – £300 Excisional skin biopsy – £600 You will be informed of an accurate pricing structure for your treatment following your consultation.

    Excellent treatment guided by Dr.Nicholas Kalavrezos and the Anaesthesia Team led by Dr Martinez. It was mg first time under General A and I wasn’t too excited. From start to finish all the nurses were superb, the room was clean and my food was also yummy. Sometimes it pays to sort your Private Medical Insurance our.     Had a FUE hair transplant on 16 December with Dr Furqan Raja.He and the two nurses he worked with were fantastic!They made me feel very comfortable throughout the procedure and constantly checked in with me. I had 1700 grafts taken from the back of my head and put into the front where I had gone bald either side of my windows peak.It took a bit longer than I thought (about 8.5 hours) but this was due to the high quality, attention to detail and best result they wanted to achieve for me.I had the procedure done at the Leeds clinic but could have chosen Manchester too. I travelled two hours there and back in one day.I am now following the after care information they gave me and also the full kit of products they provided (all included in the price) to help me make sure I look after the new grafts properly.I have follow up appointments with the nurse and access to an emergency on call nurse 24/7 if required. But so far, I have no pain and everything is exactly as described to me in advance of the procedure.I did a LOT of research online and found many of the other national FUE/FUT hair transplant clinics too sales driven and very pushy. Dr Furqan Raja and the team at the private clinic gave me time and space to decide on what was right for me, and importantly they never sold to me but just answered all my questions in a professional, caring and trustworthy manor.This is why I chose them and looking at the early results I am pleased I did!If you speak to other clinics and do your research, make sure you also book a consultation with The Private Clinic.     All of a sudden I noticed these veins under my eyes, it was soooo upsetting to see everyday! I didn’t think I could do anything about them.       Fitzroy Square Hospital

      Fitzroy Square Hospital is our purpose built day case facility for all our day case procedures. Situated on Fitzroy Square in central London makes it a quiet and peaceful location for our patients to visit. It is easily located from Warren Street or Great Portland Street tube stations. If you are travelling by car Fitzroy Square is located just off Euston Road (the A501) which runs circular around central London and there is a car park located near the square.

      Our Fitzroy Square Hospital opened in November 2016 and boasts a state of the art and modern environment with all the latest technology. There are 2 consultation rooms and 2 theatres along with recovery units for the patients to recover post-surgery before they are allowed to safely leave the facility.

      All the operations and procedures performed here are day case procedures which means the patient does not need to stay overnight and can return home the same day as their surgery to recover.

      Procedures include;

      Vaser Liposuction

      Vaser Liposuction is performed under local anaesthetic and generally takes between 1-2 hours depending on the number and size of the areas being treated.

      Breast Augmentation

      Breast augmentations are performed under general anaesthetic with the procedure taking on average an hour and a half. Some surgeons may offer you the choice of having your procedure under local anaesthetic, more so when the implant is being placed above the muscle but this would need to be discussed with your surgeon.

      Other breast procedures that can be performed here include removal of implants and replacement of implants.

      Minor Surgical Procedures

      Inverted nipple correction is a minor procedure that can be performed under a local anaesthetic with the procedure taking approximately 30 minutes for both sides to be treated. Other minor operations such as Earlobe correction, cyst removal and mole removal can also be performed here.

      When should I see a dermatologist?

      There’s more to your skin than you’d think. As the largest organ of our body, it’s an average of around two square metres when stretched out…that’s a lot of skin.

      It’s not surprising then that so many of us have problems and oddities we want to sort out. We’ve all got little quirks and it’s virtually impossible to have completely ‘perfect’ skin, but when do you need to see a specialist? We explore.

      Moles

      The average person has between 15-40 moles on their body (and many people much more). The vast majority of these are completely safe and absolutely no cause for concern, but the only way you know this for sure is by checking thoroughly and regularly.

      While seeing a professional to do this is extremely wise and strongly recommended, it’s impossible to go every single week. Therefore, we advise checking your own skin regularly in the first instance to spot any signs of trouble. The ABCD rule was devised to help spot any signs of danger and can be followed at home as frequently as you’d like – it outlines the key indicators for troublesome moles including size, texture, colour and more.       If you notice anything at all unusual with your moles or if you’re generally concerned about your risk, seeing a specialist as soon as possible is the best way to prevent any danger. Dermatologists are trained in looking out for potentially risky moles and can take a look at anything that concerns you, as well as removing it and sending it for a biopsy. While beauty salons can sometimes provide services for removing purely aesthetic moles, we strongly advise to only allow a trained dermatologist or skin doctor do so. While you might not think there is anything wrong with it, a specialist knows what signs to look out for and will ensure it is removed correctly.

      Scars

      While the initial complaint might have disappeared, scars can be a nasty reminder of acne, previous wounds or other skin issues. They are generally an aesthetic issue above anything else so very rarely need to be treated unless you want them to be. However, it is always vital to work with someone extremely well practised, so you don’t end up disappointed or – worse- with more scar tissue than before. The type of treatment you require will often come down to the type of scar you have and before recommending any specialist, we’d normally have a chat with you to make sure we choose the right one. Dermatologists are an extremely wise choice for scarring, but specialist doctors can also provide treatments. Similarly, for very small acne scarring, chemical peel or microneedling treatments may be recommended.

      Acne

      Acne affects 80% of teenagers and 10% of adults. Yet, despite being a regular occurrence, it’s one of the biggest confidence killers for a lot of us. There are, however, a multitude of treatments available to target the issue, with very few requiring you to see a dermatologist in the first instance.

      At The Private Clinic, we normally recommend meeting one of our fully-qualified skin therapists. They have extensive experience with acne so can help you understand why you’re experiencing it and what treatments will work for you. Popular methods include in-house laser N-Lite treatment and chemical peels, often paired with a specially designed home care package. If you have particularly severe acne or the team don’t think usual methods will work for you, they will then refer you to a dermatologist who can take a deeper look at your skin and see what, exactly, the best way forward is.

      Eczema and Psoriasis

      Eczema is a fairly common skin complaint, affecting around 1 in 5 children and 1 in 12 adults. Psoriasis, meanwhile, is less common with around 2% of the population suffering. Because the conditions affect people in different ways, we usually recommend a dermatologist consultation for both, but it’s important to remember that neither condition can ever be fully ‘cured’ as such. Treatment, instead, will help improve symptoms and stop any flare ups. It can often include things like emollients or topical steroids, but the method will always be individual to you. This is why it’s so vital to see the right skin expert from the start.

      Other skin complaints

      Dermatologists are able to see you for virtually anything relating to your skin, however, they aren’t always your first port of call. For some treatments, we’d always advise meeting with a one straight away (for example wart/skin tag removal or un-diagnosed skin complaints), whereas for things like anti-ageing or rejuvenation options you’ll normally be directed to a therapist, nurse or cosmetic doctor.
