Surgical Facelift Options

As we age, our face is responsible for most signs of ageing – a part of our body also most exposed. In the natural ageing process, our skin loses its elasticity and volume. Combined with gravity, this, unfortunately, causes our face to draw downwards, nearer the bone, often causing unwanted sagginess and pesky folds and lines! To help turn back these effects of time, a range of surgical facelift options are available, giving us incredible youthful and natural-looking results. Facelifts represent the most advanced and refined youth restoration procedures available, allowing you to obtain substantial, impressive improvements.

  1. What is a surgical facelift?

A facelift, or rhytidoplasty procedure, is the most comprehensive and appropriate answer to the ageing process and tissue laxity. Softening the features of the face, the procedure restores a fresher, younger look, and creates a tighter and ultimately ‘lifted’ appearance.

With a facelift, major folds and wrinkles of the face can be corrected, excessive sagging skin can be reduced and jowls can be removed. Soft tissue loss from the cheeks can also be corrected and the fallen comers of the mouth, that give a tired and sad look, can be elevated. And how does it work? In the procedure, an incision is made, and skin and tissue ‘lifted,’ then repositioned, and any surplus discarded. Absorbable stitches can then be used to close the incisions.

As we discuss, there are different variations of the procedure available, targeting different areas. In a surgeon facelift consultation, you can be advised on what is the best treatment for your individual requirements.

  1. What are the different facelift types?

There are several forms of surgical facelift available. This is in terms of the areas targeted and technique used. In past decades facelift techniques have evolved greatly and still, there is a considerable variety of preference amongst surgeons, in which techniques are used, and this, in part, also reflects on where the scars are placed.

Regarding areas targeted, at The Private Clinic we have options to target all major areas. Full Facelift-Achieve a drastic improvement to your whole face, including a more youthful, fresher appearance

-Can improve lines between the nose and mouth, surplus skin in the jowls area, and heavy or low brows, for example. Any sagging below the jaw can be corrected via another incision below the chin

-Typically, an incision is made in the temple, which runs across the hairline, then round to behind the ears, prior to lifting both the lower and upper face, and removing surplus skin, deep lines, and folds. Scarring is therefore usually along your hairline and around the areas, so scars can be hidden by the hair.

One Stitch Facelift

 

-Focuses on correcting the jowls and neck

-The treatment is the best choice for those with some jowls and experiencing moderate skin laxity

 

 

– Less invasive than a full facelift and is carried out under local anaesthetic.

– The results are more subtle in comparison to a traditional facelift but still can provide a firmer, tighter and more rejuvenated appearance.

Forehead and Brow lift (Browplasty)-Lifts the forehead, elevates brows and corrects deep lines

-Also improves the overall look of your face

– This depends on the individual patient, but in some cases, incisions are within hair-bearing skin on the scalp, or above the brow
Mid-Face Lift (Malar/ Cheeklift)-Restores both the lower eyelid region and cheeks

-Lifts sagging cheeks and gives the face a rejuvenated look

-Can also correct deep nasal folds

-The incisions lines for the Mid-Face Lift are usually hidden in the hair

 

Facelift and Blepharoplasty  (Eyelid Surgery)-A combination procedure which improves the eyelids and face overall-The incision lines/scarring involved in the facelift areas above.

-For a blepharoplasty procedure, incisions are made along the upper or lower eyelid, or both, depending on your needs

*Please note that although a facelift leaves some degree of scarring, expert surgeons will ensure facelift incisions are carefully planned to be well hidden and inconspicuous. Most scars are also concealed by the hairline. Scars will also gradually fade with good care and will become flat, making them nearly unnoticeable. Be cautious of unprotected sun exposure and smoking, to prevent scars worsening. Unprotected sun exposure causes scars to thicken and darken and smoking greatly interferes with the healing and final quality of the scars.

You are also able to combine a facelift with non-surgical treatments, to achieve particular results. For example, we recommend for some patients, a facelift combined with lipostructure (fat grafting to the face), which can both be carried out at the same time. This can fill the defects caused by volume loss, which can have outstanding results. For example, the hollow of the cheek and under eye area can be greatly rejuvenated with fat grafting. Our industry-leading, highly qualified surgeon, Mr Roberto Uccellini, who specialises in Facelift Surgery and is located in our London Harley Street, often combines facelift procedures and fat grafting at the same time under local anaesthetic.

Other ancillary procedures, such as dermal fillers, laser resurfacing and Botox can also be used in combination with Facelifts.

  1. How long is the procedure?

In regards to the procedure day, each of the facelift options discussed will last between 1-3 hours, with those that are more extensive taking longer. Many require local anaesthesia, although, general anaesthesia is required for most full facelift patients. Depending on the procedure, an overnight stay in the hospital may also be necessary.

  1. Recovery/ Healing and Timings Involved

The Private Clinic advise that you take 2 weeks leave from work post procedure, although, this ultimately depends on the nature of your profession, and the type of facelift chosen. At your consultation, we can recommend what is best for you.

In regards to recovery and side effects after treatment, the affected area will first of all be swollen and may be slightly numb around the incisions, with some bruising. Any swelling and pain you may experience, should, however, have settled down significantly in the second week. For those without absorbable stitches, stitches will be removed after 7 days or sometimes, 2 weeks, depending on the patient.

By 2-4 weeks post procedure, any bruising will be cleared, the ‘tight skin’ feeling should have improved, and any pain should now be minimal.

At a month post procedure, swelling should have reduced by 90%. Your skin may still feel slightly numb however – this can last up to approximately 4-6 months.

For the Facelift and Blepharoplasty combined procedure, all of the above is true. However, stitches for blepharoplasty will be removed 4-5 days after treatment, and it can take up a couple of months for your eyelids to completely settle.

Your surgical team should provide you with more detailed information about the post-surgery period, and be able to answer any questions you have regarding healing and recovery, at any stage.

  1. How long will my results last?

Results from each of the facelift procedures, including the combined facelift and blepharoplasty procedure, can sustain for several years, and having a good skincare regime and lifestyle can also help lengthen the effects.

What are the options for a facelift or non-surgical facelift?

To help turn back the time on your face, several non-surgical and surgical facelift options are available, offering youth-restoring, natural-looking results.

A face or neck lift, otherwise known as a rhytidoplasty procedure, is a surgical procedure that can  tighten loose skin around your face, jawline and neck region to achieve a more youthful and rejuvenated appearance. The procedure works by first making an incision, lifting skin and tissue prior to repositioning it, and discarding any excess. There are actually five face variations of the facelift, addressing different facial areas, and these are the full facelift, mini facelift, forehead and brow lift, mid-face lift and combined facelift and blepharoplasty procedure:

  • Full Facelift: This procedure can achieve a dramatic improvement to your whole face, giving a younger-looking, naturally fresher look. An incision is commonly made at the temple, running along the hairline and round to behind the ears, before lifting the lower and upper face, and removing excess skin, folds and deep lines. The procedure can improve low or heavy brows, deep lines between the mouth and nose and any excess skin in the jowls region, for example. Sagging under the jaw can also be corrected with additional incision under the chin (Pictured right).
  • Mini Facelift: Focusing on enhancing the jowls and cheeks in the lower region of the face, this procedure is best for patients with moderate skin laxity and presence of some jowls. The incisions in the Mini Face Lift are much shorter, running from the temples to the natural crease found in front of the ear, and the procedure offers minimal scaring with shorter recovery in comparison to the full face lift.
  • Forehead and Brow lift (browplasty) – This procedure lifts your forehead, elevating the brows and removing deep lines, and will enhance the overall appearance of your face too.
  • Mid-Face Lift (Cheeklift) – A mid-face lift restores the cheeks and lower eyelid area. Sagging, tired cheeks become lifted and the face has a restored, rejuvenated appearance. This treatment can also resolve deep nasal folds.
  • Facelift and Blepharoplasty (Eyelid Surgery) – This is a combination procedure used to rejuvenate the eyelids and face.

All of these procedures take between 1-3 hours, with the more extensive procedures taking longer. Many of the procedures only require local anaesthesia, but most patients need general anaesthesia for a full facelift. Depending on the treatment, an overnight hospital stay may also be required. Facelift results can last for several years, and a good skincare routine and healthy lifestyle can help prolong the effects.

To determine the appropriate procedure for you, your Consultant Reconstructive and Plastic Surgeon should assess your skin quality, general health and facial structure. With each patient having a unique face, an expert surgeon should also tailor your surgical facelift procedure to your individual requirements.

One Stitch Facelift

As well as the surgical facelift, we are also able to offer the one-stitch facelift or 3D Facelift procedure is much less invasive than a full facelift and can be safely carried out under local anaesthetic. The results are a little more subtle in comparison and very effective in producing a firmer, tighter and more rejuvenated appearance to the face. The one-stitch facelift targets the mid-face but can also help to improve the jawline. The temporal facelift is performed in a similar way but this option instead targets the upper face.

Both procedures are performed under local anaesthetic and have minimal downtime with low risk of bruising and swelling meaning the majority of patients are back to work within 1-2 days.

Non-Surgical Facelift

Not everyone is willing to have extensive surgery to turn back the years. So we’ve suggested some non-surgical facelift options that can also provide excellent results, including the Thread Lift, 8 Point Lift, Thermage, Face Fat Transfer and Dermal Fillers. These are all walk-in-out procedures, with no or minimal downtime, and usually take under an hour to complete, aside from the Face Fat Transfer taking approximately 90 minutes:

  • Thread Lift: This revolutionary method lifts sagging regions of the face with no need for incisions! The technique uses small biodegradable sutures fitted with bi-directional cones, which work to raise loose tissue on sagging cheeks or jowls, resolving wrinkles, deep lines and a loss of facial volume as well as provide jaw line definition. Offering results lasting up to 18 months, the procedure is able to target the lower jaw, eyebrows, cheek and neck. At The Private Clinic, we offer two variations of the Thread Lift – the PDO Thread Lift and Silhouette Soft Thread Lift.  Thread lift treatments are available in London Harley Street, Birmingham, Bristol, Buckinghamshire, Leeds, Manchester and Northampton.
  • 8 Point Lift: The specialised lifting and volumising treatment uses high quality dermal fillers in 8 key facial areas to lift ageing features and rejuvenate the face: the tear trough, mid-face, cheek structure, nasolabial folds, lip corners, pre-jowl area, jawlines and cheek fillers. Also referred to as the ‘liquid’ or ‘fluid facelift’ the procedure uses a precise technique to tackle soft tissue loss, and provides a more complete solution than singular filler treatments. Results for this procedure also last up to 18 months. The 8 point lift is available in London Harley Street, Birmingham, Bristol, Buckinghamshire, Leeds, Manchester and Northampton.
  • Thermage: Offering more subtle and natural-looking results, this technique uses radiofrequency energy for tightening loose areas, by stimulating natural collagen production. The procedure can target the majority of the neck, facial and décolletage areas, and effects can last for several years. Thermage is available in London Harley Street.
  • A Face Fat Transfer is a great option for those missing volume on the face. The most ‘natural’ facelift option, results are particularly soft and minimal chance of infection or scarring exists. The procedure uses minimally-invasive Vaser Liposuction to remove stubborn areas of fat from any donor site you wish. The fat then becomes refined and injected within the face, to achieve volume and lift hollow areas. Treatment Results can last for several years. Face fat transfer treatment is available in London Harley Street.
  • Dermal Fillers present a popular non-invasive solution for resolving wrinkles and fine lines, contouring the face and restoring volume to facial areas hollowed due to ageing. The procedure involves a small amount of filler, containing hyaluronic acid, being injected into the skin via an ultrafine needle. A few of the corrections possible with dermal fillers include filling out Naso-labial folds (nose to mouth lines), filling in the hollowed area under the eye, adding volume to and defining the cheek and lifting and defining the jaw line. The effects of the treatment should last for approximately 4-8 months but will gradually fade. At this point, you can choose to have a top up treatment, or if you choose to discontinue injections, the dermal filler gel will simply biodegrade and disappear. Dermal filler treatment is available in London Harley Street, Birmingham, Bristol, Buckinghamshire, Leeds, Manchester and Northampton.
  • Ultherapy is a non-surgical skin tightening procedure that is suitable for the face, neck, chin and eyebrows. Using micro-focused ultrasound with visualisation (MFU-V) energy the treatment is ale to target depths of the skin not achievable by any other non-invasive cosmetic device. The energy stimulates the production of new collagen and elastin to reverse the visible signs of ageing and provides a significant lift to the layers of tissue which other laser are not FDA cleared to treat. Ultherapy is available in London Marylebone and Stoke Poges, Bucks.
  • CoolSculpting to the chin is another great option for those looking for minimally invasive procedures for the face. CoolSculpting is a fat reduction treatment and is suitable for use on the chin. Excess fat on the chin can cause us to look older than we are and reducing this area of fat can instantly change the shape and overall appearance of the jawline. CoolSculpting does not require any needles or anesthesia. It uses controlled cooling to selectively target and destroy only the fat cells that are being treated. The fat cells are then broken down and removed by the body over time. Treatment time can take as little as 35 minutes and there is no downtime meaning you will be able to return to your everyday activities included work and exercise immediately after the procedure. CoolSculpting for the neck is available in Leeds, Manchester, Bristol, London Marylebone and Stoke Poges, Bucks.

Do I need a Mini Facelift or a Full Face Lift procedure

A facelift has the power to produce one of the most dramatic transformations of all cosmetic procedures.

It can give you a naturally fresher, younger-looking appearance which can sometimes leave people struggling to work out what in particular has changed.

But like any surgical procedure, it can only be as effective as the surgeon performing it, meaning looking for the perfect surgeon is the key to achieving a well performed and natural looking procedure.

It is so important that you find a surgeon who specialises in facelift surgery rather than one that if offering the best price.

Once you have picked your surgeon, your next task is to work out which type of facelift to go for, a full or mini?

What is the difference between a full and mini facelift?

The difference between the two techniques generally comes down to their invasiveness. A traditional full face lift usually involves a long incision that starts at the temple, and curves around the ears and hairline.

A full facelift is the best option if you are looking to improve your whole face. The procedure focuses on both your upper and lower face improving heavy or low brows, those deep lines between your nose and mouth and excess skin around the jowls area for example.

If you have sagging under your jaw, the incision can also run under the chin area. The skin is then lifted, pulled back and any excess trimmed away.

A full facelift can result in a dramatic improvement to your whole face. You may feel that you are not quite ready for a procedure with such a drastic change, or may you just feel that you are only concerned about improving one of two areas of your face, in which case a mini facelift would your best option.

A mini facelift focuses on improving the cheeks and jowls in the lower face. The incisions for this procedure are far shorter and run from the temples down to the natural crease in front of the ear. The scarring from the surgery is typically minimal and it requires a shorter recovery time compared to the full face lift.

One Stitch Facelift

Another option for a facelift is our one stitch facelift technique. What about my neck?

Another good way to decide if you are suited to a mini or full face lift surgical procedure is to look at your neck. The mini lift is essentially a face lift that just focuses on the midface and jowls. A face lift however, also focuses on the neck. The neck lift will involve incisions behind the ear, as well as a tiny incision under the chin. The ideal candidate for a mini face lift is someone who has minimal to no issues in the neck area. Any excess tissue in the jowl area and issues with skin laxity and small amounts of excess skin and fat can be made significantly better with a mini lift. If you have noticed significant banding in the neck area, also known as turkey neck and general excess skin on the face and neck area then this would be best treated with a facelift, which adds in the neck lift too.

Deciding on your best course of action for a face lift is at a surgical consultation with your surgeon. The surgeon will be able to look at your face and listen to your concerns. From here a more informed decision can be made that is personal to you and your face. It is also important to be realistic about your expectations and always ask to see before and after photographs from your surgeon to ensure that you are making the right decision.
