How to treat varicose veins?

Varicose veins are veins that have become enlarged, swollen, twisted and more prominent in appearance. They are often blue or purple in colour but can also be colour-less. Varicose veins occur when the valves within the veins that control blood flow become weak and instead of moving the blood along, it instead sits within the vein which results in the enlarged appearance.

For most patients, varicose veins are just cosmetic concern but for some, they can cause pain and discomfort as well as feeling self-conscious of the appearance of their legs.

Typical physical varicose vein symptoms include:

  • Swelling of the feet and/or ankles
  • Legs that feel heavy and ache
  • Burning or throbbing sensation in the legs
  • Muscle cramps
  • Dry or itchy skin

There are lots of options out there to help people relieve the pain of varicose veins such as:

Compression Stockings

Compression stockings can easily be found in many pharmacies and high street stores. They are often worn to prevent the occurrence of venous disorders such as ulcers and deep vein thrombosis (DVT) and can also help to boost circulation in the legs. They are often recommended to people with troublesome varicose veins as they can reduce pain and aching in the legs which is associated with varicose veins.

The relief from compression stockings will only be temporary however and there can be side effects from wearing them for too long so it may be worth discussing usage with your GP or pharmacist before purchasing.

Exercise

Exercise is often cited as the healing tool for most conditions but in the case of varicose veins it really can help. Regular exercise will encourage blood circulation in the legs which helps with the flow of blood through the veins. We recommend low impact exercises that focus of the calf muscles so walking, cycling, yoga and swimming are all good options to include into your routines.

Keeping moving throughout the day is also going to help keep the blood flowing through the veins so taking regular breaks from sitting down at a desk for example is ideal.

Diet

Diet is another common healing tool for many conditions but having a healthy diet full of the nutrients that your body needs is always going to help. If you are experiencing swelling, then reducing your intake of salty or sodium-rich foods which retain water is going to help to improve this. Instead, replace these with potassium rich foods which are known to reduce water retention so adding nuts, potatoes, lentils and leafy greens into your diet will hopefully reduce swelling.

Keeping to an ideal weight is always going to help with varicose veins complications as excess weight will put pressure on the veins so reducing this will alleviate swelling and discomfort.

Massage

A gentle leg massage can work wonders to reduce any discomfort and pain and it can also be great for encouraging circulation in the legs again. We do advise that you avoid pressing directly onto the veins however as this can damage fragile tissues.

Elevation of the legs

As if we need an excuse to put our legs up. – If your legs are feeling uncomfortable then try resting with your legs elevated, ideally at the same height as the heart or above it as this will help to improve circulation. If you know you are going to be sat down for long periods, then elevating the legs is encouraged to help relieve pain.

Clothing choice

The type of clothing or shoes that you wear could be causing the symptoms of varicose veins to become worse. High heels and tight jeans should ideally be swapped for flat shoes and loose-fitting clothing as tighter clothing can restrict the blood flow causing additional discomfort.

Surgical Treatment

All of the suggestions above can really help to reduce the pain and discomfort of varicose veins on a temporary basis, but for more permanent relief; surgical varicose vein removal treatment is going to be the best option.

Phlebectomy is usually used for large veins that are close to the surface of the skin. Small incisions are made, and tiny hooks are inserted to remove the vein. Do I need to treat my varicose veins?

Varicose veins usually develop when the small valves inside the veins stop working properly. They appear as lumpy or bulging veins usually on the legs and they may be dark purple or blue in colour. Varicose veins affect many people in the UK but many people choose to ignore their veins rather than seeking treatment. Varicose veins are not only an unsightly nuisance but they are also potential health hazards putting you at risk of blood clots, ulcers and swelling as well as venous disease.

What are the Consequences of untreated Varicose Veins?

Many patients who don’t treat their varicose veins will just experience continued symptoms of pain, fatigue and swelling of the legs or ankles. But some patients can go on to develop advanced medical problems including hyperpigmentation, lipodermatosclerosis, venous leg ulcers, spontaneous bleeding, superficial thrombophlebitis, and a potentially life-threatening condition called deep vein thrombosis

While not all varicose veins are dangerous, they should be evaluated and monitored by a vein specialist to ensure that treatment is advised when symptoms for any of the below issues are apparent.

Skin Discolouration / Pigmentation

Varicose veins that have had a period of time to progress untreated can lead to pigmentation appearing as brownish or blue skin discoloration.

Blood Clots / Superficial Thrombophlebitis

Varicose veins can cause blood in the veins to pool leading to stagnant blood flow which then clots in the insufficient vein. These blood clots can then travel to other parts of the body blocking blood flow which can then result in serious medical issues. Superficial thrombophlebitis is also caused by blood clots within the veins which can result in redness with painful hard lumps sitting beneath the skin.

Ulcers

Varicose veins can cause venous ulcers or open sores which can be painful, cause your leg to swell and are often difficult to heal. Open sores on the legs can result in infection so we recommend getting both these and your varicose veins treated as soon as possible to reduce the risk of this.

Spontaneous Bleeding

Untreated varicose veins have an increased risk of sudden bleeding. This is because over time they can become bigger and there is a high amount of pressure which can cause the veins to spontaneously burst and bleed heavily. The high pressure involved in varicose veins can cause the bleed to be very dramatic and in some cases a stitch or other medical treatment to ensure that the patient does not continue to loose what could be a large amount of blood.  It does not take much to cause spontaneous bleeding, simply brushing against the edge of a piece of furniture or even scrubbing your skin too hard in the bath or shower can cause a varicose vein to burst.

Lipodermatosclerosis

This skin condition occurs most commonly on the lower leg affecting people who have a venous insufficiency. Lipodermatosclerosis is when there is an inflammation in the layer of fatty tissue beneath the skin. It can cause your skin to become red, painful and swollen with hardening of the skin.

Deep Vein Thrombosis / Pulmonary Embolism

If you have varicose veins then you are unfortunately at a greater risk of developing DVT (deep vein thrombosis). DVT (Deep Vein Thrombosis) is when a blood clot forms in a deep leg vein, it can cause pain and swelling and can lead to more severe conditions such as a pulmonary embolism. A pulmonary embolism occurs when a section of the blood clot breaks off and works its way into the bloodstream and blocks one of the blood vessels in the lungs. Together, DVT and pulmonary embolism are known as venous thromboembolism (VTE).

What is the best treatment for varicose veins?

There are a number of different treatments available to treat varicose veins but choosing the best one for you can be difficult. From surgery and injections to lasers and glue – there are so many options out there but it is important to know that different types of varicose veins respond better to different treatment and that there is no one-size-fits-all solution, what treatment may be best for one person may not necessarily be the best for another.

To find out what the best treatment for varicose veins is for you, we recommend having a consultation with one of our specialist vascular surgeons who will be able to assess your properly with an ultrasound scan before suggesting  which treatments are going to give you the best results.

Here at The Private Clinic we do not use or recommend surgical vein stripping to treat varicose veins. This is on guidance from The National Institute of Clinical Excellence (NICE) who suggested that the technique should not be used unless it was exceptional circumstances. The range of minimally invasive options that we offer here at The Private Clinic far outperform vein stripping and there are so many better treatments available to treat varicose veins than older methods traditionally used in the NHS.

We offer a range of treatments that we believe are the best for removing varicose veins;

EVLA(Endovenous Laser Ablation)

EVLA (Endovenous Laser Ablation) uses laser heat to collapse the wall of the vein and destroy the cause of the problem. It is minimally invasive, performed in-clinic, has a quick recovery time and is considered the Gold Standard of treatment for varicose veins by the National Institute for Clinical Excellence (NICE). This is the most popular vein removal treatment here at The Private Clinic and the success rate after five years for EVLA is 95.4%. VenaSeal

VenaSeal is a closure system that uses medical adhesive to close the varicose veins. Medical grade Sapheon Glue is used to close the vein which results in the blood being re-routed through other healthy veins located deeper within the leg causing the varicose veins to then be absorbed naturally by the body. This treatment is very minimally invasive and is a great option for patients who have a needle phobia as there is a limited amount of injections required. VenaSeal also boasts a very quick recovery time with many patients not needing to wear compression garments for more than 24 hours post-procedure and most are able to return to work  and exercise within a few days.

Foam Sclerotherapy

Foam Sclerotherapy is an injectable treatment where the veins being treated are injected with a foam solution which pushes the blood out of the area and helps to seal the vein in the process too. It is a very quick procedure often taking less than 30 minutes. The treated varicose veins will begin to disappear within a couple of weeks although multiple treatments may be required.

Phlebectomy

Phlebectomy is a minor surgical procedure performed under local anaesthetic which requires making small incisions to allow the varicose veins to be removed. A hook is inserted into the tiny incision which then hooks onto a section of the varicose vein which is then removed. This procedure is great for larger bulging varicose veins and provides instant results. The incisions do not require stitches and most patients are able to return to their daily activities excluding strenuous exercise within 24 hours.

  1. EVLA: Standing for Endovenous Laser Ablation, EVLA is a highly recommended laser treatment for varicose vein removal, with a success rate after 5 years of 95.4% according to NICE. And how does it work? The treatment uses laser heat to collapse and destroy the varicose veins from the inside.
  2. VenaSeal: This is a closure system using safe medical adhesive to close the vein. Often desired by needle phobic patients, the treatment is quick and painless with shorter recovery, and there is no-need for compression stockings post-treatment.
  3. Sclerotherapy: The treatment involves the injection of a chemical into the varicose vein, which damages it and leads it to close off. The affective vein subsequently shrinks and disappears. Schlerotherapy can come in either foam or liquid form. Liquid sclerotherapy is most appropriate for small varicose veins that are not associated with underlying venous reflux and foam sclerotherapy is more suitable for larger varicose veins.
  4. Phlebectomy: A minimally invasive surgical treatment, Phlebectomy treatments involve a hook being entered via very small incisions to the affected area (using local anaesthetic), followed by the vein being gently removed. Incisions are so small that stitches are not required.

How do I choose a clinic or surgeon for my Veins treatment?

  • Ensure that your varicose vein treatment would be carried out by a Vascular Surgeon
  • Having great impact on your results, make sure to find out the surgeons experience, including how long the vascular surgeon has been performing the treatment, and how regularly they perform it. Find out if the surgeon is board certified in the field of vascular surgery or received a vein-treatment associated certification from a respected organisation. Also, check the surgeon is qualified in treating your particular condition.

  • Verify that preliminary evaluations are taken place before treatment, involving an imaging technique (such as ultrasound technology) to understand the location of your veins and check for any serious conditions
