How much fat is required for fat transfer?

 Most of us have said it at some time or another: “if only I could take fat from here and just move it here”. Not everyone, however, realises there’s been a treatment designed to do just that since the 80s.

The seventh most popular surgical procedure last year, Fat Transfer is a ‘natural’ alternative to implants and fillers which uses pockets of fat extracted through liposuction to increase volume in the breasts, buttocks, face or hands. But how much, exactly, can be transferred?

The answer isn’t simple. Each area differs massively and every single person has a unique experience of the treatment; from the amount of fat that should be harvested to begin with, to how their body reacts to the injected volume and how much it retains.

Breasts

First things first: you’re never going to be able to go from A cup to F cup through the treatment alone. As much fat as you may have to extract, if you’re looking for a guaranteed rocket in size, the method simply won’t give you the results you desire.

The treatment works best for those patients looking to increase their cup size by roughly one to one-and-a-half sizes, or those simply wishing to restore volume and fullness to sagging or empty breasts, but it’s important to remember that your body is unique and your doctor may advise on a different outcome for your case.

Despite filtering fat prior to transfer to ensure only the best is used, we find that after breast fat transfer roughly 40% of the transferred matter may be lost within the first two months, making it crucial for you to have enough fat for extraction to counter this. If you’re a very thin patient with little to be taken through liposuction, you may find that the treatment simply isn’t worth it for you; both in results and for your own health, but only a qualified and experienced doctor will be able to work out the right amount to deposit to make sure you’re happy with the results.

Bum

Like treatment for the breasts, buttocks fat transfer needs to account for the 40% loss of volume that will take place so you’ll need adequate pockets of fat elsewhere to be suitable. This doesn’t mean you have to be overweight to have the treatment; in fact, we recommend all patients be at (or at the very least near) their ideal weight. Losing weight after treatment can affect results as the fat works like any other in your body and gets lost during periods of exercise or diet.

Again, like grafting for the breasts, buttocks fat transfer isn’t the best option for a huge increase in volume, though this matter comes down massively to the skill of your doctor. At The Private Clinic, for example, our doctors will usually combine the treatment with sculpting of the flanks and thighs to give a better contour and shape to the area surrounding the buttocks, making the result a more holistic one with definition and focus all around the area.

Face and Hands

Fairly obviously, the amount of fat needed for transfer to the face or hands is not as extensive as that needed for the buttocks or breasts. It is, however, still entirely individual and there’s no way of knowing the exact necessities for you without consultation with an experienced doctor first.

Most patients come to us for the treatment as an anti-ageing alternative to restore volume so, again, the amount of fat required all comes down to the condition of your face or hands, and what you wish to achieve. It won’t be suitable for all candidates and, if your skin texture is poor, no amount of volume will give you the results you desire. The only way to know for sure is to speak with a doctor.

The women injecting their own fat to look younger

Looking down at the exquisite diamond twinkling on her finger, Rosetta Citton’s heart sank. The 59-year-old jewellery designer had slipped on the eternity ring to show one of her customers, but felt distraught at the stark contrast between the glittering stone and her ageing hands.

“That was the moment I knew I had to do something” says the married mother-of-three from London. “My hands looked gaunt and gnarled. My tendons were raised and the skin was as shrivelled as tissue paper. I had to find a way to change them – for my business as well as myself.”

But rather than opt for hand creams or fillers, Rosetta chose to have a cutting-edge procedure that sounds as if it’s straight from the pages of a sci-fi novel: a fat graft, where fat was sucked from her thighs and injected into her hands to plump them up.

Billed as a natural and long-lasting alternative to implants and fillers, these fat grafts – also used to plump up sagging boobs, or a bottom that has lost its perkiness – have become big business since being pioneered in the U.S. In the past year, the British Association of Aesthetic Plastic Surgeons has reported a 15 per cent increase in the number of treatments.

The two-in-one procedure, which costs from £1,500 to £15,000, involves removing anything from 20ml to 200ml (a third of a pint) of fat from most commonly the inner thigh or stomach, slimming the area slightly in the process. It’s then injected into somewhere lacking in volume, such as the breasts, buttocks, eye area or hands.

The treatment requires only local anaesthetic and patients are discharged the same day. While a certain amount of the newly-added fat gets reabsorbed by the body, most stays in place, ageing and depleting naturally alongside other body cells. Scarring is kept to a minimum as the skin need only be pierced with a small syringe.

As fat is softer and more fluid than synthetic fillers, the results from fat grafting look and feel natural, and there’s no risk of rejection. It stays in its new ‘home’ unless you lose weight quickly, in which case it behaves exactly like normal fat and gets broken down and reabsorbed. As a result, effects can last ten years or more compared to fillers which typically need replacing after 12 months.

Rosetta opted for the extraordinary procedure after visiting Harley Street’s The Private Clinic, where four years earlier she’d had fillers injected into her face. She arrived expecting the same treatment, but aesthetic surgeon Dr. Dennis Wolf suggested a £1,850 fat graft might give better results.

Dr Wolf has already treated three times as many people this year as last for fat transfers to their hands. Despite the fact she was conscious throughout, Rosetta says she wasn’t squeamish: ‘It didn’t hurt save for a slight burning sensation as the anaesthetic first took hold.’

First, local anaesthetic was injected into the upper back part of her thighs and hands. Fat is usually taken from somewhere that the patient has excess fat to lose; on thinner people (Rosetta is a size 8) that’s usually the thigh.

A small incision was then made in her skin and 20ml of fat – about four teaspoons – was sucked out and deposited into a machine which seperates healthy fat cells from ones damaged during extraction. The purified fat cells were then injected into the back of Rosetta’s hands. She was in and out within 90 minutes and back at work in four days. Eighteen months later, she’s still delighted with the results.

“My hands still look ten years younger. They have volume, seem healthier and smoother and the wrinkles are reduced. I don’t see it as an act of vanity as it’s natural; you’re just taking fat from one place you don’t need it and moving it to somewhere you do.”

Dr Wolf estimates that 40 percent of fat injected won’t survive past the first month. “The fat cells can die if they aren’t near enough to a blood supply to sustain them, so we always over-fill to compensate”, he says. This can cause initial swelling which usually settles down within days or weeks depending on how much fat has been transferred.

‘Does my bum look big in this?’ Anyone who answered ‘yes’ to the eternal question a decade or two ago will know it wasn’t exactly the right answer back then. Super slim supermodels like Kate Moss were the vision of perfection in the 1990s, and thin was in when it came to the female figure. Take a look at today’s most celebrated female stars, however, and it’s not hard to see that in 2014 you’re more likely to get the cold shoulder if you tell someone their behind isn’t sizeable.

The hourglass shape is back with a vengeance and celebrities like Kim Kardashian, Nicki Minaj, and Iggy Azalea all wear their curvaceous derrieres with pride. Perhaps it’s no surprise then that the number of buttocks augmentations through implants has risen 484% over the past two years, with more women opting to replicate the stars’ famous figures.

Fat Transfer
Another effective way is through a procedure known as fat transfer. The treatment, which is non-surgical and performed under local anaesthetic, increases volume in the area through pockets of your own fat which has been removed from other areas of the body.

Fat transfer has rocketed in popularity over the past few years, and last year alone The Private Clinic saw the number of fat transfers to the buttocks triple. The fat that is used is initially removed through minimally-invasive forms of liposuction, MicroLipo or VASER Lipo, before it is refined and then selectively injected into the buttocks. Does fat transfer to the breasts really work?

If you’re contemplating whether to have a cosmetic procedure or not, there’s probably two important factors on your mind. First and foremost should always be your safety and the second, how much difference the results will make to you. It’s only natural to wonder whether you’ll like the outcome, especially with a minimally-invasive treatment that doesn’t put you through as much discomfort as more traditional techniques.

Buttock Augmentation Guide: Brazilian Butt Lift vs. Implants vs. Silicone Injections

With illegal silicone injections still making headlines as vulnerable people turn to backstreet dealers to inject something that should never be injected outside of carefully controlled clinical trials, it’s important we all talk about the options and what they mean for you. Below is a guide to what you can do to enhance the size and shape of your buttocks safely and successfully.

Brazilian Butt Lift aka Fat Transfer to the Buttocks

Unwanted fat is carefully extracted from common fat deposits, like the tummy and thighs, refined and injected into the buttocks to increase the volume for a ‘natural’ boost in size. VASER Lipo can be used to extract the fat (other fat removal procedures are available). There are more options – you can upgrade the procedure to include additional liposuction for maximum body sculpting.

Pros: More ‘natural’ as it uses your own tissue, minimally invasive with minimal downtime if VASER Lipo  is used, permanent fat transfer, quick, very little scarring, general anaesthetic not always necessary, no overnight hospital visit required and it’s suitable for men and women

Cons: choose an established doctor or surgeon with a track record you can see so that you know the fat has a very high chance of staying in the buttocks. An inexperienced or bad doctor/surgeon will damage the fat cells during extraction and transfer so your body will just flush the fat away from the area. You will also need a top up eventually if you wish to retain the same results – perhaps after 10 years. Although the procedure is permanent the body is dynamic and changes as we age.

VASER Lipo

Not many people think of just having VASER Liposuction or MicroLipo but your problem may not be the lack of fat or size in the area but excess fat around the buttocks. Lipo around the upper leg and buttock crease can significantly boost and shape the area. It’s often useful to discuss this option in a consultation as our VASER doctors can also perform Fat Transfer to the Buttocks – allowing you to have a full discussion about the possibility and if you could benefit.

 Buttock Implants

Buttocks implants aren’t very popular in the UK but it still gathers some, often just initial, interest. Most buttock implants are soft solid silicone polymer implants as the area is a much more animated region under more pressure. Silicone breast implants are gel silicone which helps the implants feel like normal breast tissue. As a result of the movement and pressure involved in having buttock implants shifting is more common.

However, it can be used more effectively for medical plastic surgery to correct medical issues with the buttocks. It can be the only option for some patients so loo to a good plastic surgeon who will minimise the risks and complications. It is not a popular procedure in the UK and it is not widely available, as patients prefer less invasive procedures and treatments.

Silicone Injections

Just Cons: Google the news reports about illegal silicone injections to quickly see why you shouldn’t even consider having them. The cosmetic use of injectable liquid silicone is not approved by the US Food and Drug Administration (FDA) – a leading body for medical standards. Clinical trials for it are in their infant stages and none have been completed. However there are widespread reports and evidence of people permanently scarring themselves with silicone injections.

People often acquire liquid silicone by claiming it’s for retinal detachment but as the FDA have stated liquid silicone for cosmetic uses should not be used outside of controlled clinical trials. The American Society for Aesthetic Plastic Surgery (ASAPS) note that silicone should never be injected into the breasts as it cannot be removed without affecting the surrounding tissue. The British Association of Aesthetic Plastic Surgeons (BAAPS) caution that complications including migration and inflammation are highly likely and that the practice is explicitly banned in most countries.
