We discuss the approach of the NHS Glasgow to treating a range of physical issues, including varicose veins, acne and oversized or asymmetrical breasts. Some issues, which may be considered restraining, with physical and or mental effects or risks, are sometimes interpreted as cosmetic or periphery issues by the NHS, and treatment is limited, or unavailable.

The NHS offers restricted treatment for varicose veins, largely regarded as a periphery issue. For both complicated and uncomplicated varicose veins, compression stockings is often recommended as the primary, first line treatment. For more advanced disease cases, the NHS will advise higher grade compression (limited by the patient’s tolerance and ability to reapply the stockings daily). For veins with moderate skin changes, severe chronic venous insufficiency cases, experiencing significant skin changes or healed or active ulceration, your doctor will consider referring you for vascular assessment, as varicose vein surgery may be suitable.

The issue with trialling compression stockings, however, is the suffering of the patient is lengthened; meanwhile the condition could worsen and also develop to be more serious. It’s also important to note that varicose vein surgery offers a lower success rate and higher recurrence rate than other treatments, such as the minimally-invasive EVLA procedure, a highly effective varicose vein treatment available at The Private Clinic, and with a 95.4% success rate after 5 years according to NICE.

Breast reduction with the NHS is again limited. The procedure is unavailable for cosmetic reasons only, or to resolve breast, back, neck or shoulder pain, and cannot be used to reverse typical ageing or post-involutional changes.  It will only be considered if the patient has asymmetry of more than one cup size, if there is asymmetry after breast cancer treatment, large disproportion to body habitus, or intractable intertrigo. Patients will often need to be seen by a Clinical Psychologist prior to a surgeons assessment and BMI restrictions are in place. So patients not meeting requirements will need to look into private options for breast reduction surgery.

The NHS offer limited topical and oral acne solutions. For patients with mild to moderate acne, topical treatments usually will be prescribed and reviewed. Oral antibiotics tend to be prescribed in only extreme cases, due to risk of patients building tolerance to the medication. For cases of severe cystic acne with scarring, with severe psychological upset, and who have responded poorly to at least six months of either one of two antibiotic types and topical therapy, systemic isotretinoin, also known as Roaccutane, may be advised. Although, patients recommended the latter must be referred to an NHS dermatologist and take medical tests potentially involving long wait times. It is important to note that some acne medications involve strong side effects and have varied success rates.
